Breaking News: Mission Residency to Accept 20 Additional Candidates for Match 2021! Seat Reservations begin July 8th, 2020!

For Press Release: July 4, 2020 (New York)-  Mission Residency announced today that they’ll be accepting 20 additional Match Trainees for the upcoming 2020-21 Match Season.  After filling & closing classes this past April, this is positive news for Foreign/International medical graduates that have were unable to secure a seat for other sessions.2020 Priority List

While Mission Residency classes have historically sold out early during previous seasons, the 2020 enrollments were even more limited in part because of the Covid-19 epidemic.  That, combined with lead mentor Dr Brian Mark & his wife expecting their 3rd child on June 25th (they had a little baby boy!), resulted in more competition for less seats.

“More students isn’t always a good thing”, stated Dr Brian.  “One of the lessons I’ve learned over the past decade of training our docs is that that personal attention MATTERS when training our docs to Match.  Quality over Quanity.  It’s what our students deserve”.

Reservations for the seats will go LIVE on the website on Wednesday, July 8th @9am EST*.   Two course training levels will be available:  360Advanced (August start) & IV Prep Complete (September start).   Seats are expected to fill quickly.

The Most Frequently Asked Question by IMG’s: “Can I Match with these Red Flags?”

Question: “Dr B, I’m an IMG + OLD GRAD with low/ok scores {nsert professional history here, including USMLE Scores, YOG, Visa Status, Research, Attempts, & USCE}.  Do you think I have a chance to Match or should I give up?

It’s the question I get ALL. THE. TIME. It’s not a bad question, I just don’t think most love the answer to it.  Ready?

Answer: I have nooooo idea.

That’s right, after 11+ years of helping docs Match, when someone gives me Stats & CV info, there is no honest way to answer that question other than ‘I don’t know.’  So why do doctors I’ve never met before take the time to ask me?  Simple: they have family & friends with all kinds of red flags recommend asking for my advice because, well, they were students of ours that successfully Matched.

Is your CV an old car with a lot of miles or a brand new Lamborghini?

Asking the above question is like calling up a racing professional & asking if you can win upcoming races with the car you built.  There’s no way to know for sure.  Why?  You can have $10,000 worth of upgrades under the hood of a car, but if you don’t know how to drive it, you could easily lose to someone with minimal upgrades, but has the experience/knowledge to drive it to its full potential.

In the world of the Match – what you and I worry about every day?  I cannot tell you how many times I’ve had doctors with 210 and below on the Steps beat out candidates with 250 and above.  Or how cool it is to see an IMG that graduated medical school almost 20 years ago beat out a recent graduate for residency spot. 

So to go back to the main question, the only way to answer it with an answer that actually HELPS is to make sure ALL IMG’s understand that there are TWO sets of factors that determine the Match every March.  That’s right. There are TWO PARTS to the Match.

Part 1: PRE-September (aka BEFORE applying): The factors that matter the most here DETERMINE HOW MANY INTERVIEWS YOU GET.

Part 2: POST-September (aka AFTER applying): The factors after September that matter the most DETERMINE HOW HIGH (or LOW) PROGRAMS PUT YOU ON RANK LISTING.  THIS ultimately decides the Match.

How can your “old, high-mileage CV”  BEAT a “Lamborghini CV”? 

Ask Program Directors . . . All of Them.

Realistic Scenario:

~ I’ve met docs with 10-20+ interviews fail to Match

~ And I’ve had time and time again docs with 1 interview successfully Match

To understand why, all you’d need to do is asked the Program Directors’s. All of them.  Not possible, right?  Thankfully, NRMP asks PD’s FOR YOU! Yay!  It’s called the NRMP Program Directors Survey and it’s usually conducted every other year. (2020 at the time of this writing hasn’t been released yet, assuming the Covid-19 Pandemic made it lower on the PD’s Priority List).   Below are the results taken directly from the 2018 PD Survey:

“In March 2018, the National Resident Matching Program (NRMP) conducted its biennial survey of the directors of all programs participating in the Main Residency Match®. The primary purpose of the survey was to shed light on the factors that program directors use to (1) select applicants to interview and (2) rank applicants for the Match.” ~ NRMP Program Director Survey, Introduction

Two completely different criteria – well, Step 1 is the same, but it went from being the MOST important in the Top 5 to the Least important in the Top 5.  There are FOUR other factors that MORE programs use and ALL have higher importance. All 4 interview visit-related factors  were above a 4.7, with interpersonal skills at a 4.9!!!  In comparison, no factor ON YOUR CV is above a 4.2.  Weak Step Scores?  It may sound simple enough, but just kick a*s on the first four factors & you just overcame the score deficiency.

So to answer the original question using facts drawn directly from Program Directors? Can you Match?  Yes, if you can make the MOST out of your interview opportunities.  If you are average or below average on any of those Ranking factors, then you better have a really amazing Step score.  That’s not me saying that, that’s Program Directors saying that.

Am I biased?  Yes, my main focus for over a decade has been helping our doctors develop Top Notch interview & communication skills.  I’m biased towards that . . . because it’s what the Match Algorithm & Program Directors are biased towards when determining your Match chances.

Your CV, LOR’s & PS put you in the Race.  Interviewing WINS the Race.

And regarding the first factors for GETTING Interviews – I am in no way discounting their value.  USMLE is important – take a Kaplan Live Course, do USMLE World, get your MTB on.  Invest in teaching hospital-based USCE for LORs that matter.  Sh*t, get your arranged marriage EARLY and make sure they have a US Citizenship/Greencard.  Rocking those factors means higher quality & quantity of interviews.  The point I make to my own students & all IMGs out there – if you’re doing everything you can to get the interview . . . are you doing everything you can to make that interview COUNT.

While Mission:Residency may not have space to train more students – we believe in quality over quantity – as an advocate for IMG’s (I am one myself…and so are my closest friends), it’s sooo important for EVERY SINGLE IMG to get EXPERIENCED help in improving your rank list position.  You’ll do anything to save a patient’s life . . . are you willing to do the same to save your dream of residency that allows you to do that?

UPDATE: March 29, 2020 – Mission Limiting Students, Closing Reservations

In response to the Covid-19 Pandemic, we’ve requested that all reservations/enrollment for upcoming scheduled Mission Residency Match Training be closed for the foreseeable future.  

Before contacting Mission Residency worried, please note the following: 

~ If you have ALREADY reserved a seat, your seat is safe *** Your full season training remains as scheduled***  

~ If you’re one of our alumni that didn’t apply or had an application that could not secure interviews/Match last season, we’ll still going to guide/train you.  

~ If you’ve inquired directly with Mission via phone or email, match@missionresidency.com, there will be a short grace period that still allows you to still go on the website to reserve before closing.  You do not need to email us to check if a class is full or closed- if the system allows you to add a course to your cart, then it’s hasn’t been shut down yet.

~ For docs we cannot take in for training: We will still host free events to give advice every now and then for IMG’s that ARE NOT Mission Docs.  We just won’t be doing it as frequently as in the past. Click on Events to sign up for notifications.

Why this extreme measure during a period, when historically Mission gets the MOST enrollments/sells out sessions?

One Word: Responsibility.

Two things can never be replaced or given back: Time & the lives of, well, everyone.

Limiting enrollments at this juncture is the most responsible thing we can do to allow us to help everyone we possibly can. WHY are we taking this action? – taking less students means Allowing the time to:

1) Actively Continue to Contribute to Covid-19 Solution: As many of you know, we’ve been aggressively pursuing/petitioning to allowing all available, unmatched qualified IMG’s the opportunity to get into hospitals.  In addition to that, protecting the docs that HAVE Matched already that may be stopped because of Visa Issues.  This isn’t just Mission Docs – this is EVERYONE.  And for anyone that thinks this petition HELPS Mission Residency, it doesn’t: requesting the government find a way to get ALL qualified unmatched doctors into hospitals means 5000+ students that won’t worry about interviewing in the fall.  (it’s like a USMLE training company fighting to make all the Steps Pass/Fail, with how high scores are not mattering as much)

2) Our own Mission Docs, both past & present: 

Past – When Mission Alumni get into residency, the one thing we/re saddest about is that we can’t help & protect them the way we could when they were training for interviews.  The solution was to make sure we trained them in a way where what they learned went wayyy past interviewing skills…they were communication and life skills that would continue to be useful throughout their residency and careers.    Now, we have an opportunity to help get them MORE help in the form of unmatched docs.  (See Reason #1)

3) Present Students for 2020-21 Match Season: We have a responsibility to give them the quality time they each deserve – and have come to expect.  Between advocacy for ALL IMG’s & taking care of our families (mentioned below), taking in less students means we don’t have to compromise on the quality of time with each doc.  They won’t be penalized because we’ve taken on additional responsibilities.  

We also need to ramp up interview prep EARLIER, in the likely case that programs need to fill vacant or added slots BEFORE the September application process.

4) Family: We have family that depend on us that are stuck at home.  For Dr Brian, in addition to having to take care of a 3 1/2 year old daughter & her 1 1/2 year old sister home 24 hours a day/ 7 days a week because of school/daycare closings, Dr Brian’s wife Kate is, at the time of this decision, 6 1/2 months pregnant, due in July 2020.  They have no outside help as of now, so cutting enrollment means more time to help our students AND his wife/children.

Dr Brian’s students will tell you that we are big believers in QUALITY over quantity – it’s something that they need to master when programs give them 5-15 minutes in an interview situation.

We will re-evaluate the enrollment suspension as this fluid situation progresses. With the peak of Covid-19 pandemic in NYC (Dr Brian’s location) expected within the next 1-2 months, which will dictate that decision.

This decision to cut off enrollment wasn’t arrived at lightly.  The last thing anyone wants to feel is helpless – which is the feeling that this pandemic has given everyone.  we’ve been given an opportunity to help on multiple fronts…and while it means hurting the business side of Mission & that we will Help Less doctors as their mentor – it also means doing our civil, human, & paternal responsibility to help as many as we can.

Transcript: Covid-19, Why International Medical Doctors CANNOT be left on the sidelines in the United States. . .

The difference between winning & losing? Whether or not you have the RIGHT people on your team.

What do these  NBA Players have in common?

1) They’re all international players that brought their talents to the United States & became superstars.

2) They all are CLUTCH players…when the game was on the line, they WANTED the F’ing Ball.

3) And they all played crucial rolls on their teams in in pursuit of World Championships.

You take away their production and by numbers alone, their teams don’t win like they did.

Oh, one other thing.  Those last 3 players  I mentioned – Ginolbi, Ming, and Gasol  – all winners – have another thing in common: Their home countries – China, Italy, and Spain . . . are LOSING not to number 23. . . . but to number 19: Covid-19.  And for all their skill, experience and, most importantly, desire, this opponent is out of their league.

If you’re listening to this, quarantined . . . worried and scared, wondering what happens if I get sick…and there weren’t enough enough doctors to help everyone. What if I told you there were 4,968 people that are completely qualified right HERE in the United States, ready, willing, and able to help save us.  Doctors that have met every single piece of medical training criteria that graduates from Columbia, Harvard, and Johns Hopkins Medical school have met. And I’m NOT talking about Retired doctors that they’re calling back to hospitals….

That’s right…Like Italy and China have ALREADY done, the US is now calling on RETIRED, Doctors & Nurses to come BACK into the hospital to help overwhelmed & understaffed hospitals.   Most of them are over age 60 years old . . .That’s right.  We’re asking the population that is MOST at risk to dying from Covid-19 exposure to go back in and put their lives at risk. 

WHY?  Who else can we ask?

Now what if I told you that on March 19th,  2020, when the pandemic was on the verge of reaching a tipping point, when what we do TODAY will determine how many live and die in the weeks, months, even years ahead.  – That those 4,968 doctors with YEARS of actual experience were told that they had to wait another YEAR to work as physicians in the United States.

Now, before a group like anti-immigration groups wonder if these highly skilled foreign-born men and woman are here LEGALLY – yes, they are.  How do we know this? 

A) Many of them are US-Born students that went abroad to train & simply coming home.

B) the remaining on work/student visas like J-1, EAD, and f-1 Student visas.  And those that aren’t allowed to work without their visa can get sponsored any residency program willing to support & process the paperwork. 

What puts American Medical Graduates (AMGs) & International Docs (IMGs) on the same playing field? The USMLE = The Bar Exam for Lawyers.

For those worried that they lack the knowledge & training to measure up to doctors that graduated from US-Based Medical Schools (otherwise knows as American medical graduates, or AMGs for short), here’s what you need to know:

US Medical Graduates are required to take the United States Medical Licensing Examination, broken down into 3 major Steps…

USMLE Step 1 – Basic/Core Medical Science – like Anatomy, Physiology & Pharmacology, to name a few subjects.  It tests the foundational sciences/concepts essential to practice medicine.

USMLE Step 2 Clinical Knowledge, abbreviate Step 2CK covers clinical knowledge regarding all specialities with respect to interpreting symptoms, narrowing down a differential list of possible conditions, diagnostic studies, and appropriate treatments

USMLE Step 2 Clinical Skills, 2CS for short- It’s a live practical exam with standardized patients that tests their ability to work & communicate with patients with a range of issues.

These are the USMLE Steps that must be passed before the start of residency training, whether you’re an american medical graduate or international medical graduate.

What puts IMG’s and AMG’s on DIFFERENT playing fields?

There’s one more exam that an AMG’s don’t have to take before they begin working as a resident in a hospital. Why? It’s an exam made to test residents.  It’s called the Step 3.

USMLE Step 3 – This 2-Day final exam tests how a candidate understands and puts into practice  concepts critical in providing the best  patient care. Put simply, based on everything you’ve learned, at the end of the day . . . do you know how to save a life?  This exam is a Pre-Requisite for the majority of state licensing boards.

AMGS don’t have to take that exam.  You know who does? International Medical Graduates that are looking to be sponsored to work LEGALLY under an h1-b Visa.   And Green Card Holders or US Citizens that don’t need the visa technically don’t HAVE to take it, it’s almost a requirement because the docs they’re competing for residency slots with HAVE it.

Covid 19 is not a Chinese Virus.  It’s not a US Virus.  It’s a WORLD Virus.

Hospitals in Italy are overrun . . . and they don’t have the advantage that we have…sitting there waiting to be put back into the game.

So why aren’t nearly 5000 doctors still jobless?

1) Program directors would love extra help, but there isn’t enough resources & funding for space, faculty & visa sponsorship

2)  They struggle with the Interview Process: When they interview, they’re given anywhere between 5-15 minutes to prove to them why the belong here.  That’s not because they wouldn’t otherwise WANT to get to know them better. . . there are simply too many qualified applicants & not enough time in the day.

Why would you want Old Graduates that completed medical school years ago? One Word: Experience.

Right now there’s a taxi cab driver in NY that could save your life…one that has infectious disease experience that’s driving people around to pay his application costs to get into residency.

There’s a physical therapist MORE qualified to treat an infectious disease case than a 1st year

medical student out of Harvard.  She’s a PT because it was the fastest way she could find to support her family while applying.  Oh, and she’s worked through the SARS epidemic back home before & knows how to deal with the stress.

IMGs have seen & treated diseases that doctors don’t really see in the hospital anymore.  Tuberculosis.  Polio….even leprosy. 

That experience matters.  When an AMG has to insert an IV or intubate a patient, on day 1 of residency, that would be the first time they would have to do it as the lead medical staff on a case.  For an IMG, they would’ve done that and more hundreds of time – on their own- with no senior attending to back them up & sometimes with no electricity or air conditioning.  I spoke to an IMG named Anu last night that has literally triaged and treated 120 patients in ONE DAY.

Their Chances to Match were lowered because they made mistakes building their application . . .

In all the years I’ve been working with IMGs, there are a few things that have always been true. 

1) If they failed any of the exam I mentioned earlier, they can struggle getting even 1 interview….even when they did pass the exam the 2nd time with flying colors 

2) They’re given very little individual formal guidance on how to navigate the Maze of residency application.  You choose ANY profession where you’re left to guess your way to certification & you’d get similar mistakes.

If the AMGs can pass the USMLE in one shot, why can’t they?  

Well, AMGs throughout Med School are literally TRAINED SPECIFCALLY for The USMLE’s. They take the Step 1 shortly after completing the Step 1 material in Medical School.   

Why is it harder for an IMG? 

  1. They haven’t seen basic medical science material since medical school, which can range from 1 to 5+ years.  What have they been doing since?  Practicing medicine or rotating at US Clinical experiences, raising families, and studying for the USMLE’s…sometimes all at the same time.
  2. When they were in med school, they were trained to pass their OWN countries medical exam – which are VASTLY different in their testing method
  3. English is often NOT their 1st spoken or written language.  For some, they didn’t even grow up with the SAME ALPHABET.   Imagine taking the exam of your life in chinese characters or russian cyrillic.  Now realize that that’s what foreign-born, non-native english speaking doctor had to do… and they STILL passed the exam.

sending them back to the war front?

This is personal for me – my 80+ year old father that practiced in New York for most of his career as an anesthesiologist, is talking about wanting to go back in to help.   If it wasn’t for his heart condition or the fact that he can’t get back from the Philippines (on vacation) because of travel restrictions, we might not stop him.  Why, because that’s doctors do.  They’re wired to help whenever & wherever it’s needed, even at the cost of their own health.

In the US Military right now, we have 65,000 immigrants fighting for the US, with about 35,000 of them being non-US Citizens. Willing to die to protect and save their adopted new home country.  We honor them because we see them as the protectors of our freedoms.

We have International’s Medical Graduates that are ready to come off the sidelines and jump right onto the frontlines . . . to protect your life, liberty and the pursuit of happiness.   If you’re wondering if that’s what’s at stake – how does quarantine feel so far?

Having had 11 years of experience getting to know thousands of these doctors on a personal level, I’m telling you right now: This is like leaving a 36 year old Michael Jordan on the bench when he’s in his prime.    Get to know these doctors & you can decide for yourself.  In the next podcast, I’m going to talk to a few of these IMGs . . .

For the last 11+ years, Mission Residency’s singular focus has been helping IMG’s Match.  Our experience has allowed us to develop training that goes BEYOND simply preparing you for interviews.  We’ve developed courses that DO train doctors more thoroughly for interviews than any other program, but also help our doctors grow in their communication skills in the hospital AND in their everyday interactions living in the United States.

Our classes are intentionally kept small to allow for personal attention.  We believe in DEVELOPING & GROWING our doctors on a PERSONAL Level.  Unlike other programs that never really get to know you or limit their offerings to mock interviews, as part of the Mission Residency family, you are never just an AAMC number.
